Adventure Play Silo Waiver
PARTICIPATION AGREEMENT
I am the parent/legal guardian of the child/children named below. I give permission and accept full responsibility for the child’s/children’s use of the Adventure Play Silo at Grace City Church.
I acknowledge that participation in and use of the Adventure Play Silo involves risk to the child/children (and to the child’s/children's parents or guardians), and may result in various types of injury including, but not limited to, the following: sickness, exposure to infectious/communicable disease, bodily injury, death, emotional injury, personal injury, property damage, and financial damage.
In consideration for the opportunity to participate in and use the Adventure Play Silo, I acknowledge and accept the risks of injury associated with use of the Adventure Play Silo and transportation to and from the Adventure Play Silo. I accept personal financial responsibility for any injury or other loss sustained while using the Adventure Play Silo or during transportation to and from the Adventure Play Silo, as well as for any medical treatment rendered to the child/children that is authorized by Grace City Church or its agents, employees, volunteers, or any other representatives. Further, I release and promise to indemnify, defend, and hold harmless Grace City Church, its representatives, employees, officers, board, and heirs and assigns, for any injury arising directly or indirectly out of use of the Adventure Play Silo or transportation to and from the Adventure Play Silo, whether such injury arises out of the negligence of Grace City Church, the child/children, or otherwise.
If a dispute over this agreement or any claim for damages arises, I agree to resolve the matter through a mutually acceptable alternative dispute resolution process. If Grace City Church and I cannot agree upon such a process, the dispute will be submitted to a three-member arbitration panel for resolution in accordance with the rules of the American Arbitration Association.
